POLICY STATEMENT:
i3 Pharmaceuticals requires that all returns are authorized and accompanied by a Return Goods Authorization. i3 Pharmaceuticals reserves the right to verify all returns to ensure they adhere to this policy.
POLICY CHANGES:
Any changes to this Return Goods Policy will be communicated by i3 Pharmaceuticals to Customer with thirty (30) days prior written notice.
PRODUCT ELIGIBILITY REQUIREMENTS:
1. Credit will be given for:
- Authorized, short-dated, or expired product with less than six (6) months remaining shelf life or expired product that is not more than twelve (12) months past expiration date stated on the package.
- Damaged product purchased directly from i3 Pharmaceuticals evident at time of delivery and noted on carrier’s delivery receipt that are reported to i3 Pharmaceuticals Returns Customer Service within five (5) days of receipt.
- Concealed damage claims that are reported to i3 Pharmaceuticals Returns Customer Service within ten (10) days of receipt.
- Shipping errors that are reported to i3 Pharmaceuticals Returns Customer Service within five (5) days of receipt.
2. No credit will be given for:
- Product(s) not purchased directly from i3 Pharmaceuticals or the customer’s authorized distributor/wholesaler.
- Product not returned within sixty (60) days of Return Authorization issuance as all Return Authorization Numbers expire sixty (60) days from issuance. Authorization to return Product may only be obtained once.
- Expired returns with a total value of $25 or less.
- Products which were sold as short-date, or on a non-returnable basis.
- Product destroyed by a third-party. i3 Pharmaceuticals is not responsible for third-party processing fees.
- Product with defaced, missing or otherwise illegible lot numbers, product NDC, and/or expiration dates.
- Batched/bundled returns are not accepted or credited as each originating end customer must be assigned a unique debit memo number for individual return authorization.
PRODUCT ELIGIBILITY REQUIREMENTS:
All authorized returns in accordance with and subject to the terms and conditions set forth herein will be credited at the lower of the current invoice/net price or the original invoice/net price. Returned goods transportation costs, handling fees, or processing fees are the responsibility of the customer unless returned product is due to i3 shipping errors, damaged product, or product recalls.
RETURN PROCEDURE:
- To obtain a Return Authorization (return label) and track the return, contact i3 Pharmaceuticals Returns Customer Service by email or phone.
- E-mail or fax the debit memo to i3 Pharmaceuticals’ Returns Customer Service. Please include on the debit memo: NDC, lot number and expiration date for each returnable item.
- Returns should include only the product authorized for return and should be shipped pre-paid using the i3 Pharmaceuticals provided box label(s).
- All authorized and eligible returns should be shipped prepaid to:
